    Hi Tom,
Is it a clone, or a original.
What you could try is to flash the box with a old image file, something between 2009/2010 and see if it's works.
Also what it could be is the CFE update.

Or else it will be a hard job to fix this issue, positive thinking, it could be related to the nand flash chip, negative thoughts, cpu issue.
In any case, searching with google to,
vu+ duo CPU 0 Unable to handle kernel paging request at virtual address
    Give me a bad feeling.

This is orginal. CPU is hot. JTAG is not active. I did a cfe update. Ok, I'll look for an older image.

Hmmm theoretically should jtag always be reached, even without the memory and nand flash or security chips.
Or there must be a voltage issue thinking of the 3.3v to/near the cpu related for the jtag, or the cpu stressed, or other cpu cause.
Nevertheless, an old image file may give a difference, and check at least the jtag pins with voltage, sda/scl should be giving 3v.

In JTAG pins 3,3 SDA SCL, have 3,3V. New chip did not help.

I assume that you with new chip talking bout the nand chip.
But no, since you can not make a jtag (basically it's called I2C) connection then there is some hardware issue which could be the cpu itself, or some communication issue from/to it.
Maybe a reflow, but when I have these issues with a board and the board has been used for a long period then I would suspect the cpu.
