Based on the information you've provided, it seems to me that it could indeed be caused by a faulty nand flash chip.... But, suppose there is a bad solder from/to the nand chip, or a bad connection, then it can also give this problem. So I think that it's better to check/inspect the I/O pins and dc voltage pins of the chip before thinking of replace it. That is, if it is a pin chip, one with solder balls will be difficult to measure.
